📋 Tanggung Jawab Pekerjaan

Provide medical social work services to individuals and families, including assessments, interventions, and referrals.,Develop and implement case management plans to address the healthcare needs of clients.,Collaborate with healthcare providers, community organizations, and government agencies to ensure coordinated and integrated care.,Educate and empower clients and their families on health-related issues, promoting self-management and healthy lifestyles.,Monitor and evaluate the effectiveness of healthcare programs and services, providing feedback for continuous improvement.,Maintain accurate and confidential client records, ensuring compliance with data protection regulations.,Participate in staff meetings, training sessions, and professional development activities to enhance skills and knowledge.,Represent the organization at community events, workshops, and conferences to promote healthcare awareness and engagement.

🎓 Kualifikasi & Syarat

Bachelor's degree in Medical Social Work, Social Work, or a related field.,Minimum of 5 years of experience in medical social work or a similar healthcare role.,Strong understanding of Indonesian healthcare system, policies, and regulations.,Excellent communication, interpersonal, and negotiation skills.,Ability to work independently and as part of a team in a fast-paced environment.,Proficiency in English and Bahasa Indonesia.,Valid certification or license as a Medical Social Worker, if required by local regulations.,Experience in community health programs, chronic disease management, or healthcare advocacy is a plus.
